Relationship between emotional involvement and burnout among psychiatric residents

Elméleti háttér: Kevéssé kutatott terület, hogy hogyan függ össze a nagyfokú érzelmi distresszt átélő pácienssel folytatott kommunikáció, illetve az orvos érzelemszabályozási kapacitása. Milyen mintázat esetén jön létre kiégés, és mely védőfaktorokkal előzhető meg az érzelmi kimerülés kialakulása. Cél: Az empátia és a kiégés ok-okozati kapcsolatainak vizsgálata, a fokozott érzelmi bevonódás segítő szakemberre gyakorolt hatásainak feltárása. Módszer: Feltáró/leíró keresztmetszeti vizsgálat, kérdőíves eljárással, a 4 hazai orvosegyetemen 67 pszichiáter rezidens részvételével. Maslach Kiégés Kérdőívet; az empátia mérésére Interperszonális Reaktivitás Indexet; a páciensekkel kapcsolatos attitűd mérésére a Páciens–orvos-fókuszú Attitűd Skálát (Patient–Practitioner Orientation Scale) használtunk. Statisztikai próbák: Spearman-korreláció, Kruskal–Wallis-teszt, lineárisregresszió-analízis. Eredmények: Magas fokú emocionális kimerülés a rezidensek 32,8%-ánál, deperszonalizáció magas fokban 29,9%-nál, a személyes hatékonyság csökkenése a minta 52,2%-ánál volt tapasztalható. A férfiaknál szignifikánsan magasabb (p<0,05) a deperszonalizáció. Az Interperszonális Reaktivitás Index alskálái közül az empátiás distressz mutat összefüggést a kiégés két komponensével: az érzelmi kimerüléssel (p<0,001) és a személyes hatékonyság csökkenésével (p<0,001). A páciens-fókuszú attitűd (PPOS) összefügg az empátia három komponenesével. Fordított együttjárás tapasztalható a páciens-fókuszú attitűd és a deperszonalizáció között (MBI). A distressz érzése az érzelmi kimerüléssel és a személyes hatékonysággal is összefügg. A személyes hatékonyság érzését meghatározó változók: a megélt distressz, a nézőpontváltás képessége és a személytelen bánásmód. A deperszonalizációt a páciensekkel kapcsolatos attitűdök közül a törődés, az emocionális törődés és az érzelmi kimerülés dimenziói befolyásolják. Következtetések: A páciens negatív érzéseinek átvétele a szakember emocionális kimerüléséhez, illetve a személyes hatékonyság érzésének csökkenéséhez vezethet. Az empátiás törődés és decentrálási képesség azonban meghatároz egy olyan kapcsolati attitűdöt, mely növeli a személyes hatékonyság érzését és csökkenti a személytelen bánásmód megjelenésének esélyét.
