Absztrakt
A mesenterialis panniculitis minden hasi műtéten átesett beteget érinthet. A betegség klinikai és patológiai képe nem egységes, ennek megfelelően a terápiája is más a különböző stádiumaiban. E stádiumokat egyesek vitatják, és egy betegség különféle manifesztációinak tartják őket, míg mások folyamatában látják a betegség kialakulását. Egy 65 éves férfi betegünknél két ízben alakult ki posztoperatív fibrotisáló, meseneterialis panniculitis. Az első alkalommal laparoscopos sigmaresectiót követően észleltük a teljes mesosigmát érintő folyamatot. Az ekkor végzett Hartmann-műtét után, a tervezett vastagbél-rekonstrukciót követően másodszor is kialakult a betegség, mely ekkor a teljes mesenteriumot és a mesocolon-lemezeket involválta. Az exploratio során sebészi intervenció nem volt kivitelezhető. A beteg antibiotikum, steroid, tartós parenteralis táplálás és nasogastricus szonda mellett 3 hét alatt meggyógyult. Konzervatív sikerrel kezelt esetünk kapcsán ismertetjük e sokarcú betegséget és a kezelés lehetőségeit.
