Absztrakt
Közleményünkben pectus excavatum miatt 86 fiatal felnőttön Nuss-technikával végzett elülső mellkasfali korrekciós műtétek retrospektív elemzését végeztük el. Áttekintjük az eredményeket, az előforduló szövődményeket és azon módszereket, melyekkel a szövődmények valószínűsége csökkenthető, a műtéttechnikai részleteket is érintve. A betegek életkorát figyelembe véve értékeljük a Nuss-műtét felnőttkori alkalmazhatóságát. Az elmúlt 7 év alatt osztályunkon 86 betegen végeztünk minimálisan invazív elülső mellkasfali korrekciót. A betegek átlagéletkora 21,4 év volt. Súlyos intraoperatív szövődményt nem tapasztaltunk. Fémelmozdulást négy betegnél észleltünk, a 12, illetve 53 nappal a műtét után kialakuló elmozdulás ismételt Nuss-műtéttel megoldható volt. Egy lemezt a 13. posztoperatív napon helyi septicus szövődmény miatt, egyet pedig 14 hónappal a műtét után sterilen bekövetkező jelentős dislocatio miatt távolítottunk el. Egy beteg fájdalom miatt kérte a fém kivételét. Egy, az arteria mammaria oldalágából származó intraoperatív vérzést VATS technikával, egy tüdősérülés okozta légmellet mellkasi drainage-zsal oldottunk meg. A rekonstrukció eredménye tartós, a betegek több mint 90%-a elégedett és panaszmentes. Az évek folyamán a műtéti időtartamunk és az ápolási idő csökkent. A Nuss-műtétek biztonsággal és jó hosszú távú eredménnyel alkalmazhatók fiatal felnőttek pectus excavatum korrekciójára. A rövid hospitalizáció, kisebb műtéti fájdalom, a minimálisan invazív műtét csökkent időtartama jelenti a beavatkozás előnyeit. Fokozottabb rizikót a 30 év feletti életkor és a megelőző mellkasfali korrekció jelent.
