Absztrakt

Cél: Vesedaganat és vénás tumorthrombus miatt operált betegeink adatainak feldolgozása, tapasztalataink összegzése, ennek alapján további tevékenységünk tervezése. Módszer: 1998 és 2010 között a Semmelweis Egyetem Urológiai Klinikán 21 vesetumoros beteget operáltunk, akiknél a vesedaganat a vena cava inferior tumorthrombusával szövődött. Ismertetjük a preoperatív tüneteket, a tumorstádium-beosztást, az alkalmazott műtéti megoldásokat, a szövődményeket, a posztoperatív kezelést és a túlélési eredményeket. Az átlagos vizsgálati idő 39 hónap (3–101) volt. Eredmények: 3. szintű tumorthrombus eltávolítására öt esetben májmobilizálást és egy esetben endoluminalis occlusiót alkalmaztunk. Műtéti halálozás: 9,5%. A műtét idején metastasisos betegek medián túlélése 12,1 hónap (3–19). A metastasis nélküli betegek közül három a követési időszakban meghalt, átlagos túlélési idejük 26,7 hónap (22–31) volt. Nyolc (73%) beteg él, átlagos túlélési idejük 67,1 hónap (39–101), azaz 5,6 év. Következtetések: Fenti eredményeinkkel megerősítjük azt a megállapítást, hogy a 3. szintű tumorthrombusszal szövődött vesedaganatos betegek kevésbé agresszív módon, mellkasmegnyitás nélkül is sikerrel operálhatók.
