Absztrakt

Bevezetés: A pseudomyxoma peritonei (PMP) egy nagyon ritkán előforduló sebészeti kórkép, amely a hasüreget vagy mellüreget érinti. Jellemzője a mucinosus ascites, valamint a leggyakrabban a vastagbélből vagy a féregnyúlványból kiinduló low-grade carcinoma. Pleuralis érintettség többnyire akkor jelentkezik, ha a hasi sebészeti beavatkozáskor a rekeszizmon lyuk keletkezik, vagy a tumor progrediál, vagy fejlődési rendellenesség kapcsán a has- és mellüreg között összeköttetés van. Esetismertetés: 44 éves nőbetegnél kezelt peritonealis érintettség után bal oldali pleuralis manifesztáció jelentkezett. Tekintettel a pulmo és pleura érintettségének kiterjedtségére, a radikális műtét esélyét elvetettük, cytoreductióra törekedtünk, és intraoperatív kemoterápiát alkalmaztunk. Megbeszélés: A pleuralis érintettség extrém ritkaságnak számít, kedvezőtlen prognózisú. Kezelése megegyezik az intraabdominalis kezeléssel, amennyiben arra lehetőség van, kiterjesztett cytoreductio, valamint helyi vagy szisztémás kemoterápia javasolt. Következtetés: A PMP egy nagyon ritka és rossz prognózisú kórkép. Extraabdominalis manifesztációja ritkán fordul elő. Kezelésében sokat segíthet a korai felismerés.
