Absztrakt

Bevezetés: Az infrarenalis aortaszakaszon végzett érműtétek utáni alsó végtagi ischaemiás-reperfusiós (IR) károsodás lokális és szisztémás szövődmények forrása. A folyamatban a tüdőszövet jelentős mértékben érintett. Célkitűzés: Az alsó végtagi IR károsodás hosszú távú tüdőszövődményeinek csökkentése posztkondicionálással. Anyagok és mód-szerek: Hím Wistar-patkányokon, 180 perces bilateralis alsó végtagi ischaemiát hoztunk létre. Az állatokat Áloperált, IR és Posztkondicionált (PostC, 6×10 sec) csoportokra osztottuk. A reperfusiós időtartamok alapján (24 és 72 óra) további alcsoportokat hoztunk létre. Szövettani vizsgálat, szérum szabadgyök-, IL-6-, szöveti Hsp72-szint, illetve a szöveti nedvességarány (W/D) és myeloperoxidáz (MPO) meghatározása történt. Eredmények: A detektálható tüdőkárosodás a PostC csoportok metszetein kedvezőbb képet mutat. A PostC csoport tüdő MPO aktivitása nem mutatott jelentős különbséget, a W/D arány mindkét időpontban szignifikánsan alacsonyabb volt (68% vs. 65%; 72% vs. 68%; p < 0,05). A PostC csoportokban a szabadgyök-szint (42,9 ± 8,0 vs. 6,4 ± 3,4; 27,3 ± 4,4 vs. 8,3 ± 4,0 RLU%) szignifikánsan alacsonyabb volt; az IL-6- (238,4 ± 31,1 vs. 209,1 ± 18,8; 190,0 ± 8,8 vs. 187,0 ± 14,9 pg/ml) és Hsp72-szint nem mutatott szignifikáns eltérést. Következtetés: A posztkondicionálás csökkentheti az alsó végtagi nagyérműtétek utáni hosszú távú tüdőkárosodást.
