Bevezetés: Az érsebészeti megoldásokat forradalmasító új eljárások egyre több érterületen kerülnek alkalmazásra. Minimálisan invasiv jellegük miatt az aorta sebészete jelentősen átalakult, fokozott kockázat miatt megoldhatatlannak ítélt esetek váltak hozzáférhetővé. Az új eljárások számára új fogalmakkal és módszerekkel kell megismerkednünk. Módszerek: Régen ismert sebészi módszerek új jelentőséget kaptak az endograftok beültetésének előkészítésében. Az aorta nagy mellékágainak átépítése – a „debranching” – számos változata kerül alkalmazásra azért, hogy az endograft számára megfelelő „landing”-et, azaz rögzítési zónát alakítsunk ki. Következtetés: A hibrid megoldások sebészi lehetőségeinek ismerete a technika rohamos terjedése miatt alapvető követelménnyé vált éppúgy, mint az aorta elváltozásainak komputerizált mérése – az aortaméretek systolodiastolés változásainak figyelembevételével. Szükséges a graftok biomechanikai tulajdonságainak – stabilitás, flexibilitás, a koronakialakítás, beépülés stb. – ismerete. Alkalmazásuk, a szövődmények és kezelésük lehetőségeinek ismerete nélkülözhetetlen szakmai feladat. Ismernünk kell a hagyományos, nyitott sebészi beavatkozások szerepét a változó körülmények között. Közleményünkben a „debranching” technikai lehetőségeit ismertetjük.
