Az elhízással kapcsolatos életminőség mérése: az elhízáshoz való alkalmazkodás kérdőív rövidített változatának (Obesity Adjustment Survey — Short Form) hazai adaptációja

Absztrakt

Az elhízás az egészséggel kapcsolatos életminőség jelentős csökkenésével jár egyűtt. Mivel az életminőség javulása is az adott súlycsökkentő kezelés hatékonysági mutatójának tekinthető, az életminőség operacionalizálása kiemelt fontosságú. A vizsgálat fő célja az Elhízáshoz Való Alkalmazkodás Kérdőív rövidített változatának (Obesity Adjustment Survey — Short Form; OAS-SF; Butler, Vallis és mtsai, 1999) adaptálása és pszichometriai elemzése, valamint az elhízással kapcsolatos életminőség magyarázó változóinak vizsgálata volt.

Módszerek

Keresztmetszeti kérdőíves kutatásunkban a Szent Imre Kórház Lipidológiai Profilján zajló intézeti súlycsökkentő kezelés páciensei vettek részt (n = 339; 19% férfi). Átlagéletkor 50,2 év (SD = 13,47; terjedelem: 18–85). BMI-átlag 38,6 kg/m2 (SD = 7,58; terjedelem: 25,1–79,3). Mérőeszközök: OAS-SF, önbeszámolós antropometriai adatok, az egészségi állapot önmegítélése, a kezelt betegségek típusa és száma, CES-D Depresszió Skála, Rosenberg Önértékelési Skála, a Testforma Kérdőív rövidített 14 tételes változata, Elhízással Kapcsolatos Hiedelmek Skála.

Eredmények

A konfirmatív faktoranalízis nem támasztotta alá az OAS-SF egyfaktoros struktúráját (χ2(170) = 538,5; p < 0,0001; CFI = 0,841; RMSEA = 0,080; RMSEA CI90 = 0,073–0,088). Az alternatív, hétfaktoros struktúra elfogadható adatokra való illeszkedést mutatott (χ2(149) = 304,1; p < 0,0001; CFI = 0,933; RMSEA = 0,055; RMSEA CI90 = 0,047–0,064; Δχ2 = 234,4; ΔDF = 21; p < 0,0001). A kérdőív belső megbízhatósága jó (Cronbach-α: 0,89). A többváltozós modellben a súlyfelesleg mértéke, az egészségi állapot szubjektíve kedvezőtlenebb volta, a testtel való elégedetlenség, a testsúlykontroll költségeivel kapcsolatos hiedelmek, az elhízás társas és esztétikai következményeivel kapcsolatos hiedelmek és a depresszió magyarázó ereje emelkedett ki az elhízással kapcsolatos életminőség vonatkozásában.

Következtetés

A faktoriális szerkezet feltárása céljából szűkséges az OAS-SF további mintákon történő tesztelése. A konstruktumvaliditás vizsgálata is hátra van még. Megfontolandó a mérőeszköz 95 tételes változatának adaptálása is. Az életminőség magyarázó változóival kapcsolatos eredményeink intervenciós pontokra mutatnak rá.
