Absztrakt
A szerző áttekinti a korszerű onkológiai sebészet kialakulásának múlt századi történetét. A XX. század utolsó negyedében a colorectalis carcinoma sebészetére a radikalitás kiterjesztése vált jellemzővé. Három új eljárás jelent meg a nemzetközi gyakorlatban, amelyeket a Szegedi Egyetem Sebészeti Klinikáján a szerző és munkacsoportja a colorectalis carcinoma sebészeti kezelésében alkalmazott. 1.) A subtotalis colectomia (STC) a colon kiterjesztett – a flexura lienalist meghaladó – resectiója. Nyolc éves időszakban 72 STC-t végeztek elzáródást, vagy tünetnemző szűkületet okozó bal oldali vastagbéltumor miatt. Eredményeik alapján az STC onkológiailag elégséges, egyszakaszos műtéti megoldás, ami az ileocoecalis anastomosis miatt, akut indikáció alapján is biztonságos. Utánvizsgálataik a betegek kielégítő életminőségét igazolták a műtét után. 2.) 1996. jan. 1. és 1998. dec. 31. között 108 esetben végezték a rectum középső- és alsóharmadi carcinomájának eltávolítását a teljes mesorectum excisiójával (TME) Heald módszere szerint. Az esetek 60%-ában sikerült sphinctermegtartásos elülső resectiót végezni és a lokális recidívák arányát a hagyományos technikával operált betegek kontroll csoportjában talált 14,5%-ról 6,4%-ra csökkenteni. 3.) Az előrehaladott – IV. stádiumú – colorectalis carcinomák sebészi ellátására 154 multivisceralis resectiót végeztek, 112 esetben a colon előrehaladott daganata miatt, 42 esetben pedig rectumcarcinoma miatt. Műtéteik halálozása a colon csoportban 7%, a rectum csoportban pedig 12% volt. A tumormentes p.op. szakban a colon csoportban betegeik 90%-a, a rectum csoportban pedig 95%-a értékelte a helyzetét, állapotát, életminőségét jobbnak, mint a műtét előtt. A colon csoportban 40%-os, a rectum csoportban pedig 22%-os 5 éves túlélést értek el. Betegeiknek abban a csoportjában, amelyben a tumoros colon vagy rectum mellett 3 vagy több, más hasi lokalizációt érintett a folyamat, nem volt 3 évnél hosszabb túlélés.
