A vesék korfüggő morfológiai és funkcionális változásait nem lehet fiziológiásnak tekinteni. A korral csökken a glomerulusok száma, scleroticus glomerulusok és aglomerularis arteriolák jelennek meg. A tubulusok atrophiája mellett gyakori az interstitialis fibrosis, és az időskori érelváltozások is erősen érintik a veséket. Csökken a vesék keringése és a GFR, az utóbbit nem tükrözi a se-creatinin értéke. A tubularis transzportok zavara főként a só- és vízkiválasztást érinti, hyposthenuriát okoz. A korfüggő változások patogenezise komplex, és részleteiben még nem tisztázott. A változások mindenesetre rontják a kiválasztást, és befolyásolják a gyógyszerek farmakokinetikáját. Valódi krónikus veseelégtelenségben más funkciók (eritropoetin-képzés, D-vitamin, Ca- és P-anyagcsere) is károsodnak. Az időskorban gyakoribb szisztémás betegségek (diabétesz, hipertónia stb.) miatt a valódi krónikus veseelégtelenség is gyakoribb, de az akut veseelégtelenség különféle formái is könnyebben alakulnak ki.
