A pitvarfibrilláció és a krónikus szívelégtelenség a kardiológiai betegségek két nagy csoportját alkotják, amelyek gyakran együtt jelentkeznek. A kardiális reszinkronizációs terápia egy fontos, nem gyógyszeres, eszközös terápiás lehetőség előrehaladott szívelégtelenségben, amelynek morbiditás- és mortalitáscsökkentő hatását nagy randomizált vizsgálatok igazolták. A legutóbbi európai és amerikai ajánlás javasolja bizonyos esetekben a reszinkronizációs kezelést pitvarfibrilláló szívelégtelen betegekben. A jelen összefoglaló az aktuális irodalmi áttekintés alapján elemzi a pitvarfibrilláció és krónikus szívelégtelenség együttesét, a reszinkronizációs terápia hatását pitvarfibrilláció esetén, valamint a terápiás lehetőségeket ritmuskontroll vagy frekvenciakontroll tekintetében. A szerzők részletezik, hogyan befolyásolja a pitvarfibrilláció a biventricularis ingerlést, hogyan mérsékelhető a pitvari tachyarrhythmia negatív hatása, miközben a problémakörről egy átfogó képet alkotnak.
