Dobutamine stress cardiovascular magnetic resonance imaging in patients with peripheral artery disease

Alsó végtagi artériás érszűkületben szenvedő betegek gyakran szenvednek egyúttal coronariabetegségben, de klasszikus noninvazív kardiológiai módszerekkel történő vizsgálatuk gyakran nem lehetséges vagy jelentősen korlátozott értékű. Célunk bemutatni az alsó végtagi artériás érszűkületben szenvedő betegek kardiológiai felmérésére végzett dobutaminterheléses szív mágneses rezonanciás vizsgálat biztonságosságát és eredményességét. Huszonegy, alsó végtagi artériás érszűkületben szenvedő beteget (átlagéletkor 64,3±7,7 év) vizsgáltunk prospektív módon dobutaminterheléses szív mágneses rezonanciás vizsgálattal. A 21 betegen a vizsgálat protokoll szerint elvégezhető volt. Az életkori célfrekvenciát a betegek 95,2%-a érte el. Súlyos szövődmény nem lépett fel. A képminőség az 1-től 4-ig terjedő skálán az összes bal kamrai szegmens vonatkozásában kifogástalan (medián, interkvartilis) [4 (4-4)] volt. Öt betegnél (23,8%) volt reverzíbilis ischaemiára utaló falmozgászavar kiváltható. A falmozgászavarok megítélése tekintetében a vizsgálók közötti egyezés igen jó [κ = 0,87 (p<0,0001)] volt. Az alsó végtagi érszűkületben szenvedő betegek noninvazív kardiológiai felmérése céljából végzett dobutaminterheléses szív mágneses rezonanciás vizsgálat biztonságos és eredményes módszer. Orv.
