Urgent endoscopic retrograde cholangio-pancreatography: to whom, when and how to perform?

A sürgős endoszkópos retrográd cholangiopancreatographia hatásossága sok vonatkozásban bizonyított, de bizonyos területeken még vita tárgyát képezi. A beavatkozás időzítése sem tisztázott pontosan. Az akut biliaris pancreatitisben alkalmazott endoszkópos retrográd cholangiopancreatographia, endoszkópos sphincterotomia és kőextrakció indikációja az utóbbi időben ellentmondásossá vált. Az akut cholangitis terápiájának alapja a mielőbbi epeúti dekompresszió és drenázs. A beavatkozás idejének a cholangitis súlyosságához kell igazodnia. Epeúti sérülések okozta morbiditás és mortalitás az időben elvégzett endoszkópos retrográd cholangiopancreatographiával felállított pontos diagnózissal és terápiával csökkenthető. A nehéz epeúti kanülálás az endoszkópos retrográd cholangiopancreatographia után kialakuló pancreatitis egyik legfontosabb technikai jellegű kockázati tényezője. Az epeúti kanülálás sikerességét növelő és a szövődményeket csökkentő kanülálási technikák kerülnek bemutatásra. A szervezett képzés és az endoszkópos retrográd cholangiopancreatographia kompetencia objektív mérőszámainak követése javíthatja a vizsgálat minőségét hazánkban. Orv. Hetil., 2012, 153, 1456–1464.
