A szív hormontermelő funkciójának felfedezése gyökeresen megváltoztatta a szív szerepének megítélését, amely mechanikus pumpa helyett a szervezet folyadék-elektrolit és hemodinamikai homeosztázisát meghatározó, más szervrendszerekkel együttműködő tényezővé vált. A szív által termelt legfontosabb hormonok a natriureticus peptidek, amelyek elsődleges feladata a volumenterhelés kivédése; ezt szolgálja a natriureticus, diuretikus, vasodilatator és antiproliferatív hatás. Úgy is tekintik őket, mint a cardialis, vascularis és renalis hatások integratív markereit, azaz a cardiorenalis distressz jelzőit. Szerepük ma már messze túlterjed a patofiziológiai jelentőségen, a brain natriureticus peptid és prohormonjának N-terminális része a szívelégtelenség elfogadott plazmamarkerévé vált, amelyek a diagnózis, a prognózis megítélése és a terápia vezetése szempontjából egyaránt hasznosak, s terápiás alkalmazásukra is lehetőség nyílt. Az extraadrenalis aldoszterontermelés kimutatása az elmúlt évek izgalmas felfedezése, de az intracardialis aldoszterontermelés emberben nem tekinthető jelentősnek. Az intracardialis pajzsmirigyhormon-termelést a dejodinázok aktivitásváltozása szabályozza. Az emelkedett trijód-tironin-szint szerepe felmerült a szívizom-hypertrophia kialakulásában, a csökkent trijód-tironin-termelésnek pedig valószínűleg az oxigénhiányhoz való alkalmazkodásban van jelentősége. Az epicardialis zsírszövet és a coronariasclerosis között korábban nem sejtett, sokoldalú kapcsolat mutatható ki, a zsírsejtek citokin- és adipokintermelése része lehet az atherosclerosis önerősítő folyamatának.
