Bevezetés: A rectumtumorok terápiás stratégiájának megválasztásánál kiemelt fontosságú a daganat kiterjedésének és a regionális nyirokcsomók érintettségének ismerete. Célkitűzés: A szerzők a rectum-ultrahangvizsgálat diagnosztikus pontosságát kívánták vizsgálni a rectumtumorok stádiumbesorolásában. Módszer: 2006 és 2012 között a rectumtumorok stádiumbesorolása céljából elvégzett endoszkópos ultrahangvizsgálatokat értékelték retrospektív módon. Pontosságuk megítélésénél a patológiai stádiummal való egyezést vették alapul. Eredmények: Neoadjuváns kezelésben nem részesült betegcsoportnál a T1- és T2-stádiumra vonatkozóan a rectum-ultrahangvizsgálat szenzitivitása (75%, illetve 73%) és specificitása (74%, illetve 80%) magas, ám T3 esetén már jelentős csökkenés volt kimutatható (szenzitivitás: 58%). Az N-stádium esetében kisebb egyezést találtak (45–62%). Az onkológiai kezelés után a rectum-ultrahangvizsgálat pontossága jelentősen csökkent, a hibák döntő többsége túlértékelésből (27%) adódott. A vizsgálók tanulási görbéje rövid, már 30 elvégzett vizsgálatot követően szoros egyezést láttak a patológiai és ultrahangos stádiumok között. Következtetések: A rectum-ultrahangvizsgálat elsősorban a korai (T1 és T2) rectumtumorok stagingjében nyújt nagy segítséget. Hatékonysága a neoadjuváns kezelés után csökken, ezért nem alkalmazható a downstaging megítélésére. Orv. Hetil., 2013, 154, 1337–1344.
