Absztrakt

Az elmúlt hat évtizedben csak a K-vitamin-antagonista kumarinokat alkalmazhattuk orális antikoagulánsként. A kumarinoknak szűk a terápiás tartományuk, hatásukat számos, gyakran alkalmazott gyógyszer befolyásolja, szedésük rendszeres laboratóriumi ellenőrzést igényel (INR-meghatározás). Az új, faktorspecifikus antikoagulánsok két csoportja ismert: anti-FIIa (dabigatran) és anti-FXa (rivaroxaban, apixaban és edoxaban) hatású molekulák. A szerző összefoglalja az új orális antikoagulánsok legfontosabb klinikai jellemzőit, indikációit, laboratóriumi vizsgálatuk lehetőségeit. Az antikoagulánsok legfontosabb mellékhatása a vérzés. A közlemény részletesen kitér a nem specifikus és specifikus vérzéscsillapító gyógyszerek alkalmazási lehetőségeire, különös tekintettel a súlyos akut vérzésekre, illetve a sürgős sebészeti beavatkozások eseteire. Részletezi az antidótumok alkalmazási módjait, az ajánlott dózisokat, és ismerteti az új orális antikoagulánsok inhibitoraival elért eddigi legfontosabb klinikai eredményeket. Kiemelten foglalkozik az idarucizumabbal, amely a dabigatran elsőként már törzskönyvezett specifikus antidótuma. Orv. Hetil., 2016, 157(12), 443–450.
