Although fatty liver injury can occur in a number of non-alcoholic settings, such as hepatitis C infection, drug reactions and starvation, the term Non-Alcoholic Fatty Liver Disease (NAFLD) is reserved for the liver disorder that is associated with insulin resistance and the ‘Metabolic Syndrome’ (MS). NAFLD is the rather nondescript term used for the spectrum of liver diseases, incorporating simple steatosis, non-alcoholic steatohepatitis (NASH) and cirrhosis, which histologically resembles alcoholic liver injury in the absence of alcohol abuse. NAFLD has become a common reason for ‘liver referral’ due largely to the practice of measuring liver function tests in patients started on statin therapy and the rising prevalence of obesity in many countries. Despite being such a common problem it is still commonly misdiagnosed, overlooked or ignored. Its importance as a cause of ‘cryptogenic cirrhosis’, a cause of morbidity and mortality particularly in patients with type 2 diabetes mellitus and perhaps as an independent risk factor for cardiovascular disease is emphasized in this review. The importance of understanding the pathways involved in the pathogenesis will help direct our treatment in the future and is also highlighted.
