Abstract

Increased intra-abdominal pressure during laparoscopy leads to hypoxia due to reduced blood flow. Aim of our study was to investigate whether preconditioning can reduce this negative effect of the pneumoperitoneum. Fifty female Wistar rats were used, divided into 5 groups. I: Sham operation (Sham), II: conventional pneumoperitoneum (PP), III: transvaginal pneumoperitoneum (TV), IV: preconditioning for 2.5 minutes in two cycles (Pre 2.5), V: preconditioning for 5 minutes (Pre 5). Malondialdehyde (MDA), reduced glutathione (GSH), sulfhydrylgroup (SH-) concentrations, superoxide-dismutase (SOD) and mieloperoxidase (MPO) activity, and anti-apoptotic pathway marker p-AKT level and inflammatory cytokine TNF-α were measured. SOD activity and GSH concentration were decreased in PP and TV groups comparing to Sham and preconditioning groups. MPO activity was decreased also in PP and TV groups comparing to the Sham group but in the preconditioning groups it has remained high. MDA concentration in plasma was increased in PP and TV groups comparing to Sham and preconditioning groups. There was no difference in the case of blood MDA and SH-concentrations between groups. Anti-apoptotic pathway marker p-AKT level was decreased in the TV group comparing to the sham and preconditioning groups. TNF-α level was increased in TV and preconditioning groups compared to the sham group. According to the results preconditioning can reduce negative effects of pneumoperitoneum.

Sixty three scale insect species are reported from the Mezőföld area (Hungary), a mosaic area of predominantly intensive agricultural land and scattered grassland and forested areas. In comparison, from the Kőrös-Maros National Park, dominated by natural grassland areas, only 31 species were reported. From the Mezőföld data 4 species were new for the Hungarian fauna and almost all species from Mezőföld were new for the given locality. The Mezőföld fauna could be characterised by more rare species and a lower Global Frequency Value (=higher level of species rarity), suggesting a higher overall conservation value. Scale insect species numbers show a strong negative correlation with the ratio of woody plantations. There was an overwhelming presence of a steppic scale insect species in grassland assemblages, and impoverished woody fauna of the studied wooded areas. However, there was no correlation with plant species number, with the area of natural vegetation, or with the area of loess steppe patches, which shows that the original loess step fauna is impoverished, heavily disturbed. In summary we can say, that the Mezőföld loess scale insect fauna is relatively poor, could be characterised by widely distributed, mezophilous, common species. However, it also has several important steppic elements, and therefore deserves protection.

Asthmatic inflammation during pregnancy poses a risk for maternal and fetal morbidities. Circulating T cell immune phenotype is known to correlate with airway inflammation (detectable by fractional concentration of nitric oxide present in exhaled breath (FENO)) in non-pregnant allergic asthmatics. The aim of this study was to assess the relationship of peripheral T cell phenotype to FENO and clinical variables of asthma during pregnancy.We examined 22 pregnant women with allergic asthma in the 2nd/3rd trimester. The prevalence of Th1, Th2, regulatory T (Treg) and natural killer (NK) cell subsets was identified with flow cytometry using cell-specific markers. FENO, Asthma Control Test (ACT) total score and lung function were evaluated.Peripheral blood Th1, Th2, Treg, and NK cell prevalence were not significantly correlated to airway inflammation assessed by FENO in asthmatic pregnant women (all cells p > 0.05; study power > 75%). However, an inverse correlation was detected between Th2 cell prevalence and ACT total scores (p = 0.03) in asthmatic pregnancy.Blunted relationship between T cell profile and airway inflammation may be the result of pregnancy induced immune tolerance in asthmatic pregnancy. On the other hand, increased Th2 response impairs disease control that supports direct relationship between symptoms and cellular mechanisms of asthma during pregnancy.

Abstract

Thermobifida alba is the mesophilic member of the Thermobifida genus, the genome and enzyme sets of which have not been described and published yet. Thermobifida strains are thermotolerant actinomycete, which possess wide sets of cellulose and hemicellulose hydrolysing enzymes. Previously, three endomannanases (Man5ATh, Man5ATc, and Man5AThf) of thermobifidas were cloned and investigated, and hereby the endomannanase of T. alba DSM 43795 is described. All four endomannanases belong to the glycoside hydrolase family 5, their sizes are around 50–55 kDa. Their structure consists of a catalytic domain and a carbohydrate binding module, while there is an interdomain linker region in-between consisting repetitive tetrapeptide motifs (eg.: PPTEPTD-Ta, PTDP-Tc, TEEP-Tf, DPGT-Th). The pH optima of Man5A enzymes from T. alba, Thermobifida halotolerans, Thermobifida cellulosilytica, and Thermobifida fusca are slightly different (6.5, 7.0, 7.5, and 8.0, respectively), however, the temperature optima of the enzymes were detected within a wider range of 65–75 °C. In this research, Man5ATa exhibited the lowest Michaelis-Menten constant (KM) (0.13 mM) on LBG-mannan substrate, while others shared similar kinetic parameters: 0.9–1.7 mM of KM. Despite the high sequence similarity of the investigated mannanases, they exhibit different temperature stability parameters. These different functional characteristics can be advantageous for industrial applications producing biologically active, oligomannan prebiotics under different conditions.

Abstract

Introduction:The Natural Orifice Transluminal Endoscopic Surgery (NOTES) is an abdominal operation carried out with flexible endoscopic instruments and their advanced versions via natural orifices. The NOTES causes theoretically less pain, operative stress, shorter hospitalization, quicker recovery and it is scarless. The aim of this investigation was to evaluate the feasibility and safety of transgastric small bowel resection. Materials and methods: Seven domestic pigs were investigated and entered the study; body weight 25 kg in average. General anesthesia was performed. One trocar was used for laparoscopic observations during gastrotomy and transgastric penetration with the gastroscope, later for assistance with a grasper for manipulation of the bowel loops and for the application of the linear stapling device. Then the needle knife was used to complete a 1.5 cm long incision in the gastric wall. The gastroscope was advanced into the peritoneal cavity and a small bowel loop was identified and elevated with a flexible grasper. Through the second channel of the scope a coag grasper was used to dissect the mesentery. Then a linear stapler was inserted through the assisting trocar and the resection was performed. Afterwards the bowel ends were opened with the needle knife and the stapler was reinserted for a side-to-side anastomosis. The specimen was removed via the stomach. Gastric closure was completed by laparoscopic mini-instruments through the stapler-port. All special events and all problems were prospectively documented. Follow-up was performed over 2 weeks and body weight was recorded. Then a laparoscopy was performed to document adhesions or abscess formation. Finally the animals were sacrificed to evaluate further evidence of infection or adhesions. Results: The operations were carried without complications, there was no case of letal outcome. On the 12th day the abdominal laparoscopic revision was carried out, after the revision in one case adhesion could be detected, no other kinds of complications were noticed. Conclusion: According to our results, the transgastric small bowel resection is a safe procedure, but further special instruments are needed. These experimental procedures should be evaluated carefully and critically in clinical practice.

A Marfan-szindróma a szervezet kötőszöveti állományát érintő öröklődő betegség, amely Magyarországon hozzávetőleg 2–3000 személyt érint. A betegség manifesztációi multiszisztémásak, ezért a kórismézés sokszor nehézségekbe ütközik. Az „Országos Marfan Regiszter” jelenleg közel 250 Marfan-szindrómában szenvedő beteg adatait tartalmazza, s ez a szám dinamikusan növekszik. Célok: A Marfan-szindrómás magyar személyek biológiai mintáinak, klinikai adatainak és életmódfelmérőinek összegyűjtése. Módszerek: A szelekció során olyan betegekre esett a választás, akiknél a cardiovascularis és a szisztémás érintettség, illetve a családi kórelőzmény alapján az átdolgozott Gent-nozológia értelmében egyértelműen megállapítható a Marfan-szindróma fennállása. Eredmények: Az adatbázis alapján 102 Marfan-szindrómában szenvedő személyt tartalmaz a biobank, amely 55 személy cDNS-mintáját, 102 személy genomi DNS- és szérummintáját foglalja magába. A biológiai minták mellett minden személyről nemzetközileg validált fizikai aktivitási, táplálkozási és pszichológiai kérdőívek alapján gyűjtött adatot is tartalmaz a biobank. Következtetések: A Marfan Biobank genetikai, génexpressziós és fehérjeszintű kutatások gyors megtervezését teszi lehetővé. A biobank hozzájárul a kórkép alaposabb megismeréséhez és a hazai betegek ellátásához egyaránt. Orv. Hetil., 2012, 153, 296–302.

A terápiarezisztens, súlyos Crohn-beteg gyermekek kezelésére új lehetőséget nyújt az antitumornekrózis-faktor-α-terápia (infliximab). Jelen vizsgálat célja az infliximabterápia hatékonyságának, mellékhatásainak felmérése hazánkban, a gyógyszer gyermekgyógyászati alkalmazásának engedélyezésétől a 2008. december 31-ig terjedő időszakban. A vizsgált periódusban országosan 23 gyermek részesült infliximabkezelésben. Az indukciós terápia során a betegek 5 mg/ttkg infliximabot kaptak a 0., a 2. és a 6. héten, ahol meghatároztuk a betegség aktivitását jelző mutatót (pediatric Crohn disease activity index). Az infliximabkezelés során kedvező terápiás választ 18 betegnél (81,8%) tapasztaltunk, teljes remisszióba 13 gyermek került (59,1%). A számításokat 22 beteg adatai alapján végeztük. A kezelés 6. hetére a fistulák 70%-a bezárult. A gyógyszerrel összefüggésbe hozható akut infúziós reakció 2 betegnél jelentkezett, egy másik esetben anaphylaxiás reakció alakult ki. Késői mellékhatás három gyermeknél jelentkezett. A vizsgálatunkban részt vett, hagyományos terápiára rezisztens, súlyos Crohn-betegek döntő többségénél az indukciós infliximabterápia hatásosnak bizonyult.
