Absztrakt:

Bevezetés: A gyermekkori szerzett csontvelő-elégtelenségek ritka, kezelés nélkül halálos betegségek. Egységes diagnosztikájukat és terápiájukat európai munkacsoport felügyeli. A munkacsoport bevezette a hypocellularis gyermekkori refrakter cytopenia entitást, melyet csökkentett intenzitású kondicionálással transzplantálva lényegesen jobb túlélési eredményeket kaptak. Célkitűzés: A protokollhoz csatlakozás előtt és az azóta eltelt 5 évben kezelt betegek eredményeinek ismertetése. Módszer: A 2013 és 2017 között eltelt 5 évben a Magyar Gyermekonkológiai Hálózat 8 központjában 55 gyermeket kezeltünk (súlyos aplasticus anaemia: 9, myelodysplasticus szindróma: 41, juvenilis myelomonocyter leukaemia: 5). Súlyos aplasticus anaemiában 7 esetben végeztünk őssejt-transzplantációt, egy esetben antithymocytaglobulin-kezelést, egy beteg a diagnózis előtt meghalt. Myelodysplasiában 37 esetben végeztünk transzplantációt, 4 esetben a szoros megfigyelést választhattuk. E transzplantációk 54%-a (20 eset) csökkentett intenzitású kondicionálással történt. A juvenilis myelomonocyter leukaemiában szenvedő 5 betegnél transzplantáció történt. Eredmények: A diagnózis és a kuratív kezelés között eltelt idő medián 92 (3–393) nap volt, súlyos aplasticus anaemia esetén 28 (3–327) nap. Akut graft versus host betegség II–IV. fokozatú súlyossággal 22,6%, III–IV. fokozatú súlyossággal 6,8%-ban jelentkezett, míg betegeink 11,2%-a krónikus graft versus host betegségben szenvedett. A súlyos aplasticus anaemiával kezelt 8 beteg mindegyike teljes remisszióban él (100%). A myelodysplasia miatt transzplantált betegek becsült túlélése 85,1%, juvenilis myelomonocyter leukaemiában 75%. A medián követési idő 30,4 (1,1–62,5) hónap volt. Jelen eredményeinket összevetettük az 1992 és 2012 között kezelt betegek eredményeivel. A túlélés az új szemlélet nyomán jelentősen javult, súlyos aplasticus anaemiában trendszerűen 70%-ról 100%-ra (p = 0,133), myelodysplasticus szindrómában szignifikánsan 31,3%-ról 85,1%-ra (p = 0,000026). Következtetés: Paradigmaváltás történt a gyermekkori szerzett csontvelő-elégtelenségek kezelésében, a betegcsoport túlélése szignifikánsan növekedett. Orv Hetil. 2018; 159(42): 1710–1719.

A clinicopathological case study of vertebral osteomyelitis caused by Pasteurella canis in a 2.5-year-old male Jack Russell Terrier is presented. The case was characterised by a chronic course with signs of spinal pain and acute paraplegia. The diagnosis was established by radiography, myelography, post-myelographic CT examination, and laboratory tests including routine blood work and cerebrospinal fluid (CSF) cytology, and confirmed by postmortem pathological and microbiological examinations. Diagnostic imaging showed severe osteolysis, ventral spondylosis and spinal cord compression at the 4th and 5th lumbar vertebrae. The blood tests revealed mild leukocytosis and anaemia, while CSF cytology showed lymphocytic and mononuclear pleocytosis. Necropsy demonstrated severe osteomyelitis and meningomyelitis, but the source of infection could not be established. To the authors’ knowledge, this is the first description of canine vertebral osteomyelitis caused by this organism.

Medical records of 80 dogs diagnosed with acute pancreatitis during a 4-year period were evaluated regarding history, breed predilection, clinical signs and additional examination findings. Cases were selected if compatible clinical symptoms, increased serum activity of amylase or lipase and morphologic evidence of pancreatitis by ultrasonography, laparotomy or necropsy were all present. Like in other studies, neutered dogs had an increased risk of developing acute pancreatitis. Although breed predilection was consistent with earlier reports, some notable differences were also observed. Apart from Dachshunds, Poodles, Cocker Spaniels and Fox Terriers, the sled dogs (Laikas, Alaskan Malamutes) also demonstrated a higher risk for pancreatitis according to our results. Concurrent diseases occurred in 56 dogs (70%), diabetes mellitus (n = 29, 36%) being the most common. Clinical signs of acute pancreatitis were similar to those observed in other studies. The study group represented a dog population with severe acute pancreatitis, having a relatively high mortality rate (40%) compared to data of the literature. Breed, age, gender, neutering and body condition had no significant association with the outcome. Hypothermia (p = 0.0413) and metabolic acidosis (p = 0.0063) correlated significantly with poor prognosis and may serve as valuable markers for severity assessment in canine acute pancreatitis.

Autologous vascular patch grafts developed from the internal rectus sheath were implanted onto the bilateral common iliac vein and jugular vein of 4 experimental beagle dogs. During the development and implanting of the grafts no technical difficulties or perioperative complications were encountered. The follow-up lasted 6 months and 3 months in the case of the common iliac vein grafts and the jugular grafts, respectively. In the postoperative period, the morphological and functional characteristics of the implanted venous sections were examined by Doppler ultrasonography and CT angiography. Normal patency was detected, and none of these check-ups showed obturation or stenosis. The histological survey showed no mesothelial cell layer, but the insides of the grafts showed total restructuring and were covered by a normal endothelial layer. No difference could be detected between samples harvested 3 and 6 months after implanting. The immunohistochemical examinations using anti-claudin-5 and anti-CD31 antibodies confirmed the preliminary results of the histological examinations that the luminal surfaces of the implanted grafts developed a differentiated monolayer endothelium which was free of degenerative and inflammatory signs. The control examinations show the suitability of the internal rectus sheath as a venous wall donor.

In our recent investigation, angiogenesis was evaluated and quantified by immunohistochemical evaluation of microvessel density (MVD) using claudin-5 (CLDN-5) as a marker for vascular endothelium in 67 canine mammary gland tumours. Computer image analysis was used to measure the intratumoural MVD. Higher intratumoural MVD was detected in malignant simple neoplasms compared with benign tumours. Furthermore, the results of MVD were correlated with histological grade, higher grades being accompanied by higher MVD. In simple adenomas and grade I tubular-tubulopapillary simple carcinomas the intratumoural microvessels were wide and regular in shape with evident erythrocytes in their lumen. In grade III solid carcinomas the microvessels were smaller, less regular and had irregular shape, often without a distinct lumen, and isolated endothelial cells were frequently present. In the complex carcinomas MVD was low and the intratumoural microvessels were mostly irregular in shape without a distinct lumen. The evaluation of MVD by CLDN-5 immunohistochemistry may give useful additional information on the angiogenic potential of breast cancers in dogs.

Four nozzle types were tested on large scale trials with 2500 m 2 plot size. The AIC TeeJet, the TeeJet XR, the Turbo FloodJet and the Turbo TeeJet Duo nozzles were tested in 2006 and 2007. 250 l/ha spray amount was used for all nozzles at 7–8 km/hr speed. Nine fungicides generally used to control FHB were tested. Across nine fungicides the mean reduction in symptoms was 60 % for both AIC TeeJet and XR TeeJet nozzles, but the more effective fungicides performed better with the XR TeeJet nozzle. The Turbo TeeJet Duo nozzles reduced the symptoms by 70 % and the Turbo FloodJet nozzle by 80 %. Among the fungicides the Prosaro 1 l/ha was the most effective. The coverage was controlled by water sensitive paper stripes and UV sensitive color mixed into the spray. Data show better uniformity by paper stripes, the UV pictures show that the all-side protection by the Turbo FloodJet nozzle is not perfect, the coverage is less than anticipated. Therefore, further work is necessary for improve the coverage being necessary to an even more effective control. However, the existing improvements allow 80 % or higher reduction with the best fungicides giving a chance for a better protection. Small plot tests with the same fungicides gave highly similar results achieved with the large-scale farm application. Farm efficacy could be forecasted with r = 0.90 correlation based on small plot hand sprayed tests.

Absztrakt

A Gardner-syndroma a familiaris adenomatosus polyposis klinikai altípusa, autosomalis dominánsan öröklődő betegség, amelyet a gastrointestinalis traktus polyposisa és extraintestinalis elváltozások jellemeznek, mint multiplex osteomák, valamint bőr- és lágyrésztumorok. A Gardner-syndromához társuló desmoid tumorok terápiás kihívást jelenthetnek. A szerzők egy 17 éves nőbeteg esetét mutatják be, akinél a Gardner-syndroma a lumbalis régió agresszív desmoid tumorával jelent meg. A beteget a 80 hónapos utánkövetés során sebészileg, non-steroid gyulladáscsökkentőkkel, tamoxifennel és sugárterápiával kezeltük. Következtetésként elmondhatjuk, hogy familiaris adenomatosus polyposisban vagy Gardner-syndromában a desmoid tumorok megjelenése megelőzheti a betegség gastrointestinalis manifesztációját. Az ilyen betegeknél genetikai vizsgálatot és colonoscopiát kell végezni, megelőzendő a későbbi colorectalis rosszindulatú daganat kialakulását. A desmoid tumor multidisciplinaris kezelést igényel.

Összefoglaló. Bevezetés: Nagyszámú kutatás igazolta, hogy a dohányzás növeli a legjelentősebb krónikus betegségek kockázatát. Habár 2009 óta csökkenő tendenciát mutat Magyarországon a hagyományos dohányzók aránya, az e-cigarettát kipróbálóké az utóbbi években folyamatosan növekszik. Célkitűzés: A 2018-ban a felnőtt lakosság körében végzett Népegészségügyi Felmérés dohányzásra és e-cigaretta-használatra vonatkozó eredményeinek bemutatása az előző vizsgálatok tükrében. Módszer: A kérdőíves felmérésben 1586 fő került személyesen lekérdezésre. Az iteratív súlyozás a többlépcsős mintavételi designhatást és a 2016-os mikrocenzus adatait vette figyelembe. Eredmények: 2018-ban a dohányzók aránya a magyar felnőtt lakosság körében 28,7% (95% MT: 26,3–31,1%), az e-cigarettát használók aránya pedig 1,7% (95% MT: 1,1–2,5%) volt. Az iskolai végzettség a 65 év alattiak esetében a dohányzást befolyásoló tényező volt (EH: 3,32; 95% MT: 2,53–4,34), de a 65 éves és annál idősebb korcsoportban már nem (EH: 1,11; 95% MT: 0,59–2,09). Az e-cigarettát kipróbálók és használók között a leginkább említett (54,3% 95% MT: 44,0–64,5%) motivációs tényezőcsoport a dohányzásról való leszokással, az ártalomcsökkentéssel és a visszaesés megelőzésével volt kapcsolatos. A 65 éves és idősebb korcsoportban a dohányzók aránya 2015-höz képest emelkedett. 2018-ban az alapfokú iskolai végzettségűek körében volt a legmagasabb a dohányzók aránya, míg 2014-ben az érettségivel nem rendelkező középfokú végzettségűek körében. Következtetés: Bár összességében csökkent, az alacsony iskolai végzettségűek és az idősek körében emelkedett a dohányzók aránya Magyarországon. Az e-cigarettát kipróbálók és használók száma növekvő tendenciát mutat hazánkban. Eredményeink az alacsony iskolai végzettségűekre kiemelten fókuszáló, megelőző és leszokást támogató népegészségügyi alprogramokat is tartalmazó komplex beavatkozást sürgetnek. Summary. Introduction: The body of evidence suggests that smoking increases the risk of the most prevalent chronic diseases. Although the proportion of traditional smokers in Hungary has been on a declining trend since 2009, the proportion of those who tried e-cigarette has been steadily increasing in recent years. Objective: To present – in the light of previous studies – the results of the Public Health Survey among adults in 2018 on smoking and e-cigarette use. Method: 1586 persons were personally interviewed in a survey. The iterative weighting algorithm considered both the design effect of multistaged sampling and the 2016 Hungarian microcensus. Results: In 2018, the proportion of smokers in the Hungarian adult population was 28.7% (95% CI 26.3–31.1%), and the proportion of e-cigarette users was 1.7% (95% CI 1.1–2.5%). Educational level was a predictor of smoking among respondents younger than 65 years old (OR 3.32; 95% CI 2.53–4.34), but not for those aged 65 years or older (OR 1.11; 95% CI 0.59–2.09). Among e-cigarette ever or current users, the most commonly mentioned (54.3% 95% CI 44.0–64.5%) motivational factor-group to try or use e-cigarettes included motivations to quit smoking, to reduce harm, and to avoid relapsing. In the population aged 65 years old or older, the proportion of smokers increased compared to 2015. The proportion of smokers was the highest among those with primary education in 2018, while in 2014, it was the highest among those with secondary education without a graduation certificate. Conclusion: In Hungary, although overall smoking rates are declining, the smoking rate in the low educational group and among the elderly increased. The number of people trying or using e-cigarettes is showing an increasing trend in our country. Our results call for a complex public health intervention program including prevention and smoking cessation supporting subprograms with high focus on those with primary education. Abstract

Background

Hormone level changes after heart surgeries are a widely observed phenomenon due to neurohormonal feedback mechanisms that may affect postoperative morbidity and mortality. The current study aimed to analyze the changes in thyroid and sex hormones in the first 24 postoperative hours after heart surgery.

Methods

This prospective, observational study (registered on ClinicalTrials.gov: NCT03736499; 09/11/2018) included 49 patients who underwent elective cardiac surgical procedures at a tertiary heart center between March 2019 and December 2019. Thyroid hormones, including thyroid-stimulating hormone (TSH), triiodothyronine (T3), and thyroxine (T4), and sex hormones, including prolactin (PRL) and total testosterone, were measured preoperatively and at 24 h postoperatively.

Results

Significant decreases in serum TSH (P < 0.001), T3 (P < 0.001) and total testosterone (P < 0.001) levels were noted, whereas T4 (P = 0.554) and PRL (P = 0.616) did not significantly change. Intensive care unit (ICU) hours (P < 0.001), mechanical ventilation (P < 0.001) and Vasoactive-Inotropic Score (VIS) (P = 0.006) were associated with postoperative T3 level. ICU hours were associated with postoperative T4 level (P = 0.028). Postoperative and delta testosterone levels were in connection with lengths of stay in ICU (P = 0.032, P = 0.010 respectively). Model for End-Stage Liver Disease (MELD) scores were associated with thyroid hormone levels and serum testosterone.

Conclusions

T3 may represent a marker of nonthyroidal illness syndrome and testosterone may reflect hepatic dysfunction. In addition, PRL may act as a stress hormone in female patients.

Abstract

Purpose

The frailty concept has become a fundamental part of daily clinical practice. In this study our purpose was to create a risk estimation method with a comprehensive aspect of patients' preoperative frailty.

Patients and methods

In our prospective, observational study, patients were enrolled between September 2014 and August 2017 in the Department of Cardiac Surgery and Department of Vascular Surgery at Semmelweis University, Budapest, Hungary. A comprehensive frailty score was built from four main domains: biological, functional-nutritional, cognitive-psychological and sociological. Each domain contained numerous indicators. In addition, the EUROSCORE for cardiac patients and the Vascular POSSUM for vascular patients were calculated and adjusted for mortality.

Results

Data from 228 participants were included for statistical analysis. A total of 161 patients underwent vascular surgery, and 67 underwent cardiac surgery. The preoperatively estimated mortality was not significantly different (median: 2.700, IQR (interquartile range): 2.000–4.900 vs. 3.000, IQR: 1.140–6.000, P = 0.266). The comprehensive frailty index was significantly different (0.400 (0.358–0.467) vs. 0.348 (0.303–0.460), P = 0.001). In deceased patients had elevated comprehensive frailty index (0.371 (0.316–0.445) vs. 0.423 (0.365–0.500), P < 0.001). In the multivariate Cox model an increased risk for mortality in quartiles 2, 3 and 4 compared with quartile 1 as a reference was found (AHR (95% CI): 1.974 (0.982–3.969), 2.306 (1.155–4.603), and 3.058 (1.556–6.010), respectively).

Conclusion

The comprehensive frailty index developed in this study could be an important predictor of long-term mortality after vascular or cardiac surgery. Accurate frailty estimation could make the traditional risk scoring systems more accurate and reliable.
