Abstract

We are exposing a differential diagnostic problem in the case report of a young female with the diagnosis of atrial fibrillation who was referred to our Institute for pulmonary vein isolation. Based on the 12-lead ECG we found an unusal mechanism resulting in irregular, narrow QRS tachycardia, which was also confirmed by an electrophysiological study.

In this study molecular markers linked to the Ry sto gene, which originates from the wild potato species Solanum stoloniferum and confers extreme resistance against PVY, were identified and the applicability of recently published Ry sto markers was analyzed. Three RAPD markers covering a total distance of 8.60 cM were detected in this experiment. The closest of these markers was located 0.53 cM from the gene. From among the published markers only one had diagnostic value in the experimental plant material, and mapped 2.95 cM from the gene, on the side opposite the RAPD markers developed in the present study. All the markers analyzed were present in Solanum stoloniferum accessions, irrespective of their resistance, indicating that these sequences are linked to the locus and not exclusively to the dominant allele of the Ry sto gene in the wild species. The inapplicability of several published markers indicates that the genetic background is decisive in this tetraploid and highly heterozygous species. This means that it may be necessary to develop markers from the breeding material itself, until the resistance gene is not cloned and cannot be used as a selection marker in marker-assisted selection.

Abstract

Aims: We investigated the influence of body mass index (BMI) on the prevalence of responder status in chronic heart failure patients after cardiac resynchronization therapy (CRT).

Methods: Data on 169 patients with resynchronization therapy were analyzed. Patients were categorized on the basis of the BMI measured at device implantation according to the WHO classification, as normal (BMI: 18.5–24.9 kg/m2), overweight (BMI: 25–29.9 kg/m2) or obese (BMI:≥30 kg/m2). Patients were considered responders if left ventricular ejection fraction was increased by at least 5% at 6-month follow-up.

Results: The mean age in the study population was 60.9±10.86 years (females 29%). The BMI subgroups did not exhibit any significant differences in baseline characteristics (age, gender, left ventricular ejection fraction or NYHA class). Elevated BMIs were associated with higher prevalence of responder status (overweight: 71.4%, obese: 63.0%) relative to subjects with a normal BMI (44.7%) (p=0.015).

Conclusions: In this CRT population, overweight status was associated with a more favorable response to CRT, indicating that the response may possibly be influenced by factors other than those directly related to the heart status or the technical details of the CRT.

Background

Catheter ablation is a proven therapy of focal atrial tachycardia. However limited information is available about the additional value of electroanatomical over conventional mapping methods for this specific arrhythmia.

Methods

Consecutive catheter ablation procedures of FAT were analyzed in two cardiology centres. Only conventional mapping was used in 30 of the 60 procedures whereas additionally CARTO mapping was performed in another 30 procedures. Acute, six-month success rate, and procedural data were analyzed.

Results

Localization of ectopic foci is congruent with previously published data. There was no statistically significant difference between procedure time and fluoroscopy time using additionally CARTO mapping, compared to conventional mapping only. Acute success rate was higher in procedures guided by CARTO mapping than in procedures based on conventional mapping (27/30 vs. 18/30, p = 0.0081). During the 6-month follow-up period there was a better outcome (p = 0.045) in case of CARTO guided procedures (success: 11 cases, partial success: 12 cases, failure: 4 cases) compared to conventional mapping (success: 4 cases, partial success: 18 cases, failure: 7 cases).

Conclusions

Catheter ablation of focal atrial tachycardias using the CARTO electroanatomical mapping system seems to provide higher acute and 6-month success rate compared to ablation using conventional mapping methods only.
