A nosocomialis fertőzések fő okozói az egészségügyi többletkiadásoknak és szoros összefüggésben állnak a betegbiztonság problémakörével is. Hatékony infekciókontrollal, szigorított higiénés intézkedésekkel, célzott szűrővizsgálatokkal a kórházi fertőzések 30%-a megelőzhető. Napjainkban a methicillinrezisztens Staphylococcus aureus az egyik legfontosabb nosocomialis patogén. Az ellene történő védekezés lépései jól ismertek, ezért választottuk ezt a kórokozót költséghatékonysági vizsgálatainkhoz. Kiszámoltuk a győri Petz Aladár Megyei Oktatókórházban kétéves periódus alatt lezajlott methicillinrezisztens Staphylococcus aureus okozta járvány költségeit és szembeállítottuk a szűrővizsgálatok költségeivel. Eredményeinket összehasonlítottuk a nemzetközi szakirodalmi adatokkal is. Megállapíthatjuk, hogy a szűrővizsgálatok költségei jóval alacsonyabbak, mint a kórházi fertőzések kezelésének költségei, ezért elsődleges szerepe a prevenciónak van.

Bevezetés: A terhességi cukorbetegség időben történő felismerése és megfelelő kezelése az anya és a magzat számára egyaránt kiemelkedő fontosságú. Noha nemzetközi és nemzeti szaktársasági állásfoglalások születtek az időben történő felismerés módszertanáról, az esetek egy része az Egészségügyi Világszervezet által ajánlott (1. ajánlás) s ma legáltalánosabban használt módszerrel is rejtve maradhat. Több vizsgálatban összefüggést találtak az anyai vércukorszint és a halvaszülések, a praeeclampsia-előfordulás, illetve a születési időre számított nagy súlyú újszülöttek világrahozatala között, a legnagyobb visszhangot a Hyperglycemia and Adverse Pregnancy Outcomes vizsgálat hozta. Megállapításai nyomán a Diabéteszes Terhességgel Foglalkozó Munkacsoportok Nemzetközi Szövetsége a 75 grammos cukorterhelésen alapuló, de az 1. ajánlásban szereplőtől eltérő kritériumrendszerű szűrést javasolt (2. ajánlás). Célkitűzés: A vizsgálat célja várandósok terhességi kimenetelének elemzésével a két szűrési eljárás hatékonyságának összevetése volt. Módszerek: Egyszeres terhességet hordozó 1107 várandóst, 831 normális glükóztoleranciájú és 276, a két szűrés valamelyikével gestatiós diabetesesnek minősülő terhest vizsgáltak, részletesen elemezve a terhességi kimenetel anyai (koraszülés, túlhordás, sectio caesarea, toxaemia) és magzati mutatóit (születési súly, újszülöttkori hypoglykaemia). Eredmények: A születési időre számított nagy súlyú magzatok prevalenciája kivételével – melynek csökkentésében a 2. ajánlás, illetve az ennek alapján megkezdett gondozás hatékonyabbnak bizonyult – a két szűrési eljárás eredményességében érdemi különbség nem volt kimutatható. Következtetés: Az új ajánlás szerinti szűrés a glükózanyagcsere-zavar biztonságosabb felismerését eredményezheti, az egyértelmű állásfoglalás azonban nagyobb esetszámú további vizsgálatokat igényel. Proteases constitute a significant part of cell wall-degrading enzymes (CWDEs) produced by fungal biocontrol agents and particularly crucial in mycoparasitism of fungal phytopathogens. Plate-based screening methods are routinely used for screening protease-producing microorganisms including fungi. Skim milk agar (SMA) is one of the most popular media for the detection of protease producing bacteria. However, SMA is not efficient to test fast growing fungi, because it does not give an estimation of the actual amount of secreted protease produced by fungal inocula. In the current study, the efficacy of two modified plate-screening methods, including split-SMA (SSMA) and minimal medium supplemented with skim milk (MSMW) was assessed for detection of protease production by three representative fungal strains including Trichoderma longibrachiatum strain N, Beauveria bassiana strain B and Purpureocillium lilacinum strain PL. Protease production was revealed on the three tested media by the three strains. However, the halo diameter of the fungal strains (a proxy for protease production) was the smallest on SMA. Furthermore, protease production could not be detected for T. longibrachiatum strain N on SMA due to its fast growth; while it showed the highest protease activity on both modified media compared with the other strains. According to the result of this study, the SSMA medium is an easy and more accurate method compared with the two other different methods as it displays the actual amount of protease produced by fungal strains and therefore this method is recommended for quantitative and qualitative detection of protease production by slow and fast growing fungi.

Abstract  

A simple and easy to perform method for the determination of 230 Th in soil and sediment samples using gamma-spectrometry is proposed and compared with the more usual procedure using alpha-particle spectrometry. The proposed method involves an efficiency calibration for the specific determination of this radionuclide instead of a complete calibration of the spectra. A single density parameter is introduced to correct the efficiencies obtained from the reference sources. The activities of 230 Th determined by gamma-spectrometry and alpha-particle spectrometry are compared. The minimum detectable activity values obtained are completely adequate for 230 Th determination in soil and sediment samples, in which 230 Th activities are usually fairly high. Thus, this procedure is intended as a very easy way to obtain the activity of this radionuclide in these types of samples, at least as a screening method before carrying out other more laborious procedures.

This work aims to present a rapid and precise screening method of wheat germplasm and identification of durum wheat accessions in germplasm collections. Fifty-two wheat accessions maintained in the ISOPlexis germplasm bank at the Madeira University, Portugal, and 72 accessions from the Centre for Conservation of Crop Biodiversity of Tenerife (CCBAT), Spain, have been screened for their specific ploidy status using the molecular marker Dgas44. We have demonstrated that the Dgas44 sequence is effective in the screening of Madeiran and Canarian wheat accessions. This screening method permitted the detection of 10 and 11 durum accessions among Madeiran and Canarian wheats, accounting for 19.2 and 15.3% of screened collections, respectively. The obtained results have shown a 100% of cases correspondence with the previously performed morphological identification of the Madeiran wheats. It also permitted rectification and clarification of previous classification of some accessions based only on the morphological traits. The PCR based assay was directly applicable to the screening of seeds and was suitable for detecting seed mixtures in accessions. This rapid method has been proven to be a useful tool in gene bank accessions management including verification of their ploidy status and detection of seed lots adulteration.

Summary

Transfer of four rapid thin-layer chromatography (TLC) screening methods used to detect substandard and counterfeit pharmaceutical products to quantitative high-performance TLC (HPTLC)-densitometry methods is demonstrated. These methods for acetaminophen, acetylsalicylic acid, ibuprofen, and chlorpheniramine maleate are contained in a Compendium of methods developed by Kenyon and Layloff for use in countries with limited resources. The new quantitative methods use Merck HPTLC silica gel 60 F254 glass plates, automated standard and sample application, and automated densitometry for detection, identification, and quantification. Standard and sample solution preparation and application procedures for obtaining calibration curves and bracketed samples are described. The HPTLC plates give better efficiency, selectivity, and resolution than TLC, and the new methods overcome the deficiencies in technology related to manual application and visual zone comparison that do not allow the Compendium TLC procedures to support regulatory compliance actions. These transferred methods can be fully validated according to International Conference on Harmonization (ICH) guidelines or by interlaboratory studies if their applications require. The approach described can be used to transfer the remaining Compendium methods as well as the GPHF [Global (formerly German) Pharma Health Fund E.V.] Minilab kit TLC screening methods.

Transfer of two rapid thin-layer chromatography (TLC) screening methods used to detect markedly substandard and fake pharmaceutical products to quantitative high-performance TLC (HPTLC)-densitometry methods is demonstrated using a model procedure that was published earlier. These TLC methods for diazepam and amodiaquine are contained in a Compendium of methods by Kenyon and Layloff and a Minilab method manual from Global Pharma Health Fund E.V., respectively, for use in countries with limited resources. Merck HPTLC Premium Purity silica gel 60 F254 glass plates, automated standard and sample solution application with a CAMAG Linomat 4, and automated densitometry with a CAMAG Scanner 3 were used for detection, identification, and quantification. Sample peak identity and purity validation were carried out by spectral comparison checks available in the winCATS software. Accuracy was estimated by the standard addition approach with overspotting standard and sample solutions. HPTLC gives better efficiency, selectivity, and resolution than TLC, and the new methods overcome the deficiencies in technology related to manual application and visual zone comparison that do not allow the Compendium and Minilab TLC procedures to support regulatory compliance actions. These new methods can be fully validated according to the International Conference on Harmonization guidelines or by interlaboratory studies if required by their applications.
