Thromboembolism and antithrombotic management in pregnancy

A vénás thromboembolia a terhességek hozzávetőleg 1 ezrelékében fordul elő. Jelentősége az, hogy az anyai mortalitás egyik vezető oka. A terhességgel és szüléssel kapcsolatos élettani változások kedveznek a vénás thromboembolia kialakulásának, de a manifesztációhoz egyedi kockázati tényezők járulnak hozzá. A terhességhez kötődő vénás thromboembolia leggyakoribb kockázati tényezője a korábbi vénás thromboembolia és a veleszületett thrombophilia, továbbá egyes belgyógyászati betegségek és a terhesség, illetve a szülés különleges szövődményei. Thromboprophilaxis céljára terhesség alatt a mechanikus védelmen kívül csak a heparinkészítmények alkalmasak, amelyek közül leginkább a kis molekulatömegű heparint használják. A heparinkészítmény adagolását a vénás thromboembolia kockázatának nagysága és típusa határozza meg. A várandósság alatt jelentkező vénás thromboembolia kezelésére 12 óránként általában 100 NE/kg, bőr alá adott, kis molekulatömegű heparint használunk. A szoptatás ideje alatt biztonságos megelőzést biztosít a K-vitamin-antagonista kezelés is. Orv. Hetil., 2011, 152, 815–821.
