Bevezetés: A genetikai hátterű 2-es típusú cukorbetegség előállapotának a felismerése azért óriási jelentőségű, mert a praediabetes az utolsó pillanat, amikor még életmód-változtatással és esetenként kiegészítő gyógyszeres kezeléssel a betegség manifesztációja megelőzhető. Cukorbetegek első fokú rokonai jelentik a leginkább veszélyeztetett csoportot, ezért olyan szűrővizsgálatokra van szükség, amelyek a kockázatot még időben felfedik. Célok: A szerzők diagnosztikus értékű anyagcsere-különbséget kerestek normális szénhidrát-toleranciájú férfiak között, aszerint, hogy volt-e első fokú cukorbeteg rokonuk vagy nem volt. Módszer: A vizsgálatot 73 férfinél végezték, akik közül 21-nek volt első fokú cukorbeteg rokona. Eredmények: A klemp módszerrel mért glükózfelhasználás, a glükóztolerancia-vizsgálatok a két csoportot nem különítették el, de a cukorterhelés során a cukorbeteg rokonokkal rendelkező egyénekben a szabadzsírsav-szint nem volt szupprimálható, a testtömeg és a test zsírtartalma magasabb volt, akárcsak a leptin-IL-6 vérszint, míg az adiponektinszint alacsonyabb, a szabad zsírsav/adiponektin arány pedig jelentősen magasabb. Már ilyenkor is atherogen típusú volt a lipidkép, alacsonyabb a HDL-koleszterin és a nagy molekulájú, kis denzitású LDL-frakciók vérszintje. Következtetések: Egészséges, de első fokú, 2-es típusú diabéteszes rokonokkal rendelkező férfiakban a kialakuló inzulinrezisztenciát és glükózintoleranciát megelőzi a zsírsavanyagcsere zavara. Az észlelt szabad zsírsav/adiponektin arány eltérésének mérése vagy az orális, illetve iv. cukorterhelés első és utolsó időpontjában megmért szabadzsírsav-szint férfiak esetében lehetőséget nyújt egyszerű, informatív és olcsó szűrővizsgálati módszer kialakítására.
