A gyógyszereléssel összefüggő hibák előfordulása igen gyakori. A hozzájáruló tényezők és kockázatok korlátozott ismerete gátolja az eredményes prevenciós stratégiák fejlesztését és kipróbálását. Célkitűzés: Az elrendelt és osztott gyógyszerkészítmények közötti eltérések felmérése és a kockázatok azonosítása a gyógyszerosztás során. Módszer: Prospektív, direkt megfigyeléses vizsgálat két kórházi fekvőbeteg-osztályon. Eredmények: A megfigyelt gyógyszeradagok száma 775, az elrendelt gyógyszeradagoké 806, a maximális hibalehetőségeké 803 volt. A nem megfelelő gyógyszeradagok száma 114, a hibaarány 14,1% volt. Az észlelt hibatípusok közül a nem megfelelő hatóanyag-mennyiség osztása 25,4%-ban, nem felezhető készítmény felezése 24,6%-ban, behozott gyógyszer elmaradt osztása 16,4%-ban, nem az elrendelt hatóanyagú gyógyszer kiadagolása 14,2%-ban fordult elő. A nem megfelelő gyógyszeradagok 87%-át komolyabb következményekkel nem járó csoportba sorolták be, míg a hibák 13%-a minősült potenciálisan súlyosnak. Következtetések: A direkt megfigyeléses módszer alkalmas a kórházi fekvőbeteg-osztályok gyógyszerelési gyakorlatának megismerésére és a kockázatok feltárására. A gyógyszerelés és betegbiztonság javítása érdekében a vizsgálat eredményeit, az azonosított kockázatokat a szerzők továbbgondolásra érdemesnek tartják az ellátói gyakorlatokban. Orv.
